orchestrator-agent 3de9f4b970 Add the nightly missed-quest sweep
Fires one nestquest_quest_missed event per past-due open instance at
the configured rollover time and at startup, watermark-guarded in the
new nestquest_meta_state table (migration 7, strictly additive).
Read-only over the domain tables; never reports an instance that was
ever completed.

"""Typed DAO layer for quest_instances and the append-only completion_events.
All SQL for these tables lives in this module per the feature
guardrails: callers get typed dataclasses back and never see raw rows
or SQL. Every method is async and runs through
:class:`~.db.NestQuestDatabase`, so each statement executes on the HA
executor and the event loop never blocks.
Append-only policy (Feature 08, D-005): ``completion_events`` is
strictly append-only. This module is where that property is enforced
mechanically — the completion-events DAO below exposes NO update or
delete method at all; completions append, un-completions append a
separate reversal event, and instance state derives from the latest
event. A package-wide guard test (bottom of the test module) keeps
mutation SQL for this table out of every other module.
Upsert semantics: ``QuestInstancesDao.upsert`` is idempotent on
(definition_id, child_id, due_date, window) — re-running
materialization for a tuple whose instance already exists refreshes
only the generation-time snapshot columns and never duplicates a row.
The schema's UNIQUE(definition_id, child_id, due_date, window) is the
last line of defense. The upsert refuses a child that is not an
assignee of the definition and a window outside ``const.QUEST_WINDOWS``
(D-008 multi-assignee, multi-window model).
``delete_future_uncompleted`` implements the Feature 06/07 rule that
reassignment and schedule edits regenerate future instances: it
removes only instances at or after the cutoff that have NO completion
event, inside one transaction, so an instance completed mid-call can
never be deleted. Instances in the past are never touched (Feature 07
guardrail: never delete history), and no method here deletes a single
instance that has a completion event.

The instance key is (definition_id, child_id, due_date, window)
(D-008): running materialization twice for the same tuple
yields ONE row — the ON CONFLICT path refreshes the
generation-time snapshot columns (due_time) and regenerates the
stamp, but can never create a duplicate. The schema's
UNIQUE(definition_id, child_id, due_date, window) backs this
up, so the twice-daily case (two windows, one child, one date)
yields two rows and the shared case (three assignees, one
window) yields three.
Guardrails enforced here:
- ``due_date`` must be TODAY or later: instances are never
generated in the past (Feature 07). The caller's
materialization horizon is future-dated by definition.
``today`` optionally pins the resolved HA-local date the
guard compares against (falling back to the host clock when
omitted) so a household time zone behind the host around
midnight is not misread as "past".
- ``window`` must be one of ``const.QUEST_WINDOWS``.
- An existing instance WITH a completion event is IMMUTABLE:
the conflict path must not rewrite its snapshot columns
(Feature 07/08). The upsert refuses with ValueError in that
case — silently no-oping would mask a materialization bug.
- The child must be an assignee of the definition (D-008
multi-assignee model).
The definition must exist (validated atomically under the
connection lock so a concurrent definition deactivation or
child deletion cannot slip between check and insert).

"""Insert one dated instance iff its tuple is still materializable.
This is the materialization walk's single write path. Unlike the
generic :meth:`upsert`, which validates only existence and
assignment, this method re-validates EVERY materialization
precondition in the SAME connection-lock-held transaction as the
INSERT — closing the check-then-insert gap a concurrent config
write could otherwise slip through:
- the definition exists AND is active (a definition deactivated
after the input snapshot is skipped, never written);
- the child exists AND is active (a deactivated child is skipped);
- the (definition, child) assignment link still exists (a removed
assignment is skipped);
- the window is still declared on the definition (a removed window
is skipped);
- the window's live ``due_time`` still EQUALS the caller-supplied
``due_time`` (NULL-safe: NULL == NULL counts as equal; a window
with no due time matches a snapshot with no due time). A
window-time edit landed after the input snapshot makes the
caller's ``due_time`` stale, and writing it would overwrite the
newer due_time a concurrent regeneration already upserted — so
the tuple is skipped instead of written.
The due_time check closes the one check-then-insert gap this
method previously left open: it re-validated every OTHER
precondition but then wrote the snapshot's ``due_time``
unconditionally, so a stale materialization could clobber a
newer, concurrently-regenerated due_time.
Returns the stored instance, or ``None`` when a precondition
failed — the tuple no longer materializable, which the walk SKIPS
rather than raising, so the rest of the batch is unaffected. The
no-past rule (instances are never generated in the past) is
preserved from :meth:`upsert` and still raises ValueError; ``today``
optionally pins the HA-local anchor it compares against. An
instance that ALREADY has a completion event is also skipped
(returns ``None``) rather than raised: it is immutable, so the
walk leaves it untouched instead of failing the batch — the
generic :meth:`upsert` keeps its hard refusal for the
delete/regenerate path.
